Previous employee members please use the following form to re-join the University Recreation Center. Any new employeee members must visit the University Recreation Center Main Office.

Bi-weekly, $120 divided by 26 = $4.62 per check

Statement of Responsibility & Declination of Risk I authorize TCU Payroll to deduct the amount above. This deduction will continue until I notify the Department of Campus Recreation in writing. Changes or additions can only be made at the anniversary date (the date you purchased your membership). I also acknowledge that I will comply with the rules and regulations of the University Recreation Center or risk forfeiting this membership. By submitting this form, you are agreeing to the above terms and conditions.
